HMO
patients If you are an HMO patient, you will need to first call your Health Plan and inform them that you would like to choose Dr. Huang as your Primary Care Provider (PCP). To discourage frequent switching from doctor to doctor, many HMO’s may require that their patients wait a certain period of time before they are allowed to make an appointment with their new doctor. Please, always call to check plan participation or if you have any other questions. Medical records If you have chosen Dr. Huang as your physician, please ask your previous Healthcare Provider to send us a copy of your Medical Records. Please print out this Request to Release Medical Information, complete the form and mail it to your previous Healthcare Provider. Having accurate and complete medical records is important, and helps maintain continuity of care.
