Requesting medical records


For most patients, the Yellow Immunization Record will be enough.

Records will be mailed to your doctor.  Please allow at least a week.

Write a short letter to request medical records.  Your letter must include:
1.    Parent name
2.    Patient name
3.    Patient birthdate
4.    Specify if you need just the immunization record or the entire  chart
5.    New doctor’s name and address
6.    Parent signature and date

*Please help defray the cost of mailing your records.  Please send a check to the PO Box above.  A typical medical record containing 2 years of visits costs approximately $3 to mail. 

A CLARIFICATION

Many patients have asked for a copy of their medical records to keep for themselves.  If you would like your own copy of the medical records, you will be charged.  The fee is $12, plus 25cents per page, plus postage.  Unfortunately, we can only calculate the fee after the chart has been printed.  Medical records will be released once payment is received.

If you have lost your Yellow Immunization Card, you will be charged $10 to replace it.

Nobody likes being nickel and dimed.  I have never charged patients for filling out school forms, replacing yellow immunization cards or medical records.  But you will find that it is common and standard practice to charge for these items.  Since I am effectively now going out of business, I cannot and will not continue to give away these items for free.

If you have your medical records forwarded to another doctor, you will not be charged.  This is a courtesy.  This is also to ensure that you have another doctor who will then become the custodian of your medical records.   This creates a reliable chain of custody for the medical records. 

You will have plenty of opportunity to request medical records at any time.  There is a PO Box and email address that you can use after the office closes.  I encourage you to find a new doctor, and then the medical records will be sent to the doctor free of charge.  

To sum up:

     Medical records mailed to new doctor-- no charge

     Medical records given to anyone else-- you will be asked to pay

     Extra copies-- you will be asked to pay
